Customer Rights
& Responsibilities You have the right to:
• Be treated with dignity, courtesy,
and respect.
• Receive reasonable coordination and continuity of service
from referral source to home medical equipment services.
• Receive a timely response from our company when home care
equipment is needed or requested.
• Be fully informed of policies, procedures, and charges for
services and equipment, including eligibility for third party reimbursement
and receive an explanation of all forms you are requesting signed.
• Receive home care equipment and services regardless of race,
religion, political belief, sex, social status, age or handicap.
• Receive proper identification & courteous service from
personnel providing services to you.
• Participate in decisions concerning home care equipment
needs, including the right to refuse services within the confines
of the law.
• Have all of your records (except as otherwise provided for
by law of third party payor contracts) and all communications, written
or oral, treated confidentiality.
• Access to all health records pertaining to you and to challenge
and have your records corrected for accuracy.
• Express dissatisfaction and suggest changes in any service
without fear of coercion, discrimination, reprisal, or unreasonable
interruption in service.
• Receive information on the mechanism for receiving, reviewing,
and resolving your complaints or concerns.
• Be assured that your rights are honored by all our staff.
• Be informed of your responsibilities regarding home care
equipment and other services provided to you.
Customer Responsibilities:
• Customer agrees that rental equipment will be used with
reasonable care, not altered or modified, and returned in good condition
(normal wear expected).
• Customer agrees to promptly report any malfunctions or defects
in rental equipment or supplies purchased so that repair/replacement
can be arranged.
• Customer agrees to provide access to all rental equipment
for repair/replacement, maintenance, and or pick up of the equipment.
• Customer agrees to use the equipment and supplies purchased
for the purpose indicated and in compliance with the physician's
prescription.
• Customer agrees to keep the equipment in their possession
and at the address to with it was delivered unless stated otherwise
by the agency.
